The Justice Department has released a 400 page report detailing a “pattern of serious failures” at the Bureau of Alcohol, Tobacco and Firearms in the execution of Operation Fast and Furious.  

The report says that Attorney General Eric Holder was not aware of the operation, a position he’s held since the case began. However the Justice Department watchdog has recommended that 14 employees be reviewed for “a pattern of serious failures.”
